Librarian of the Year
Information Requirements:
- Describe and document specific contributions to the profession in the past year.
- Describe and document specific contributions reflecting the library and its goals to the community.
- What specific goals, both personal and library-established, has the librarian accomplished?
- Give evidence of original thinking in programs initiated, in planning, in use of resources, and in special projects.
- List any specific talents/abilities the librarian has used in furthering his/her library career during the past year.
- Give evidence of success of library through programs or policies initiated or developed by the librarian.
- Include a picture of the librarian (preferably black and white).
Distinguished Service Award
Information Requirements:

- Describe the need for project/activity.
- Describe and document the nature of the project -- e.g., new or improvement of existing project/activity, direct public service, or internal operations.
- What service population was most benefited?
- Describe and document what was done.
- Was project or activity unique or creative or especially suited to one or more groups of patrons?
- Describe the benefits of project/activity.
- What was the cost of the project/activity -- both financially and personally in terms of time and energy required.
Milstead Award
Information Requirements:
- Describe and document the nature of the service, either new or continuing, and the extent or range of services.
- Describe the direct and indirect benefits of service.
- Describe the need for the service.
- Describe the result of the service.
- Were services unique or special to particular children or groups of children?
Trustee Citation
Information Requirements:
- Describe and document what has been done, both specifically and generally.
- Describe the effect on the community, the library, and its staff.
- Describe the actions and policies initiated or continued.
- Describe the results and benefits of actions and policies.
- Were policies or actions unique or creative or especially suited to your particular community?
Media Support
Information Requirements:
- Type of media.
- Describe and document benefits from media coverage.
- Describe level or kind of cooperation from media.
- Describe prominence and frequency of coverage given.
- Describe range of coverage.
- Describe attitude expressed by media staff.
- Give name of media staff to be especially thanked.
Meritorious Service
Information Requirements:
- Describe and document type of support given.
- Describe and document what was done.
- Describe benefits of support.
- Was support unique or creative, unsolicited or solicited?
- Was there any special cost in money or time and energy given?
Award Award
Information Requirements:
- Describe and document event, activity, idea, etc., as much as possible.
- How was it especially amazing, inspiring, unique, humorous, etc.
- What was the scope of it?
- What was the effect of it?
- What were the benefits of it?
Unsung Heroine/Hero
Information Requirements:
- Describe and document specific contributions to the library, and the community.
- List any specific talents/abilities the nominee has used in furthering his/her library career.
